Home Care Services. Personal care and assistance with household tasks; home modification to make your home safer.
Home Health Care. Certified medical and nursing services provided in the home
Hospice (in-home). Pain and symptom relief in cases of terminal illness
Home Delivered Meal Programs. Meals and/or groceries delivered to the home
Medical Equipment. Durable equipment and supplies for rent or purchase
Monitoring and Reminder Services. Vital sign monitoring; medication and Dr.'s appointment reminders via beeper or email.

Meal Programs. Meal services in a group setting
Senior Center. Community centers offering social, educational and other programs

Board and Care Home. Non-medical care in a small group setting
Hospice. Residential facilities offering pain and symptom relief in cases of terminal illness
Independent Living Community. (Continuing Care Retirement Community) Housing and services adapted to seniors' changing health needs.
Congregate Housing. Supervised group housing for the elderly and disabled, also called 'supported housing'
Alzheimers/Dementia Facilities. Specialized care for people with Alzheimers or dementia
Medical Services
Hospital. Short-term, high-level medical care
Rehabilitation Center. Specialized rehabilitative therapies
Renal Dialysis Center. Specialized services to help with kidney functions
Outpatient Services. Medical, dental, vision and dietician services; addiction treatment and pain management clinics; mental health assessment and care.
Geriatric Assessment. Specialized assessment for the elderly
Caregiver Resources
Caregiver Support. Education, advice and advocacy for family caregivers.
Grief Recovery. Helps caregivers deal with their grief and loss and move on with their lives.
Care Coordination and Other Services
AAA. Public or private nonprofit organizations providing information and referrals for senior services including housing placement assistance.
Care Manager. Finds and coordinates health care and other services
Elder Law Attorney. Specialists in legal issues facing seniors; tax issues and estate planning.
Elder Abuse Prevention. Prevent or report cases of elder abuse; ombudsman programs to investigate complaints regarding care and general well-being.
Long-term Care Insurance. LTC insurance offers peace of mind, but is it right for you?
